Who May Qualify for MLTC in New York?

The state decides who qualifies for MLTC. It is based on your Medicaid, your health, and how long you need care. We can help you check before you apply.

  • Active or qualifying New York Medicaid
  • Meet the age or program rules for the plan type you want
  • Need home care for more than 120 days
  • Pass the health check through the New York Independent Assessor Program (NYIAP)
  • Live in the service area of the plan you pick
  • Can stay at home safely with the right help in place

You cannot be enrolled in MLTC while also enrolled in NHTD, an OPWDD waiver, or a Health Home, since these are separate care management structures.

If you don't currently qualify for MLTC, Private Pay home care may still be an option.

Step-by-Step Guide

How to Enroll in an MLTC Plan in New York

How long it takes depends on your health check, the plan you pick, and your own situation.

    Confirm Active NY Medicaid

    You need active NY Medicaid with home care coverage before anything else can start.

  2. 2

    NYIAP Health Check

    A state nurse checks your daily care needs, usually by video call. Call 855-222-8350 to book.

  3. 3

    Get Your Outcome Letter

    After the visit, a letter comes in about 7 business days. It tells you if you qualify and opens up plan enrollment.

  4. 4

    Pick Your MLTC Plan

    Choose a plan in your county. Send in your paperwork by the 20th of the month to start the next month.

  5. 5

    Nurse Visit and Care Starts

    Your plan sends a nurse to set your care hours. You have 90 days to switch plans before the 9-month lock-in starts.
