  • Age 21 or older
  • Enrolled in both Medicare and full Medicaid
  • Live in a 2026 MICH service area
  • Not living in a state-operated veterans home
  • Not currently enrolled in hospice

2026 service area

MICH launched in selected counties and Upper Peninsula areas in 2026. MICH health plans are available in Wayne County and Macomb County, along with Barry, Berrien, Branch, Calhoun, Cass, Kalamazoo, St. Joseph, Van Buren, and most Upper Peninsula counties. Oakland County is not listed in the 2026 MICH service area.

Important: plan availability can differ by county. Cottage can help you understand the home-care side, but MDHHS and your MICH health plan determine enrollment, covered benefits, network rules, and service authorization.

The MI Health Link program ended December 31, 2025. MI Coordinated Health launched January 1, 2026 as the MI Health Link replacement for eligible people enrolled in both Medicare and Medicaid.
