  • Report from the Quantiferon Lab.

  • A copy of your COVID-19 vaccination card.

  • To process payments, you need either a direct deposit form or a voided check.

  • Physical exam (must have been done in the recent six months).

  • Drug Screen with 8–10 Panels.

  • You need to have booster shots for rubella and rubeola (measles).
